Enrolled students may connect with the Disability Access and Inclusion Student Services (DAISS) office if they are interested in requesting accommodations, learning more about the accommodation process, or identifying as a student with a disability.

Please note: Students requesting an Emotional Support Animal (ESA) or housing accommodations must meet with an Accessibility Counselor and follow the established ESA and Housing Request Processes.
Please describe how your symptoms affect your learning and ability to complete coursework. This may include difficulty concentrating in lectures, challenges with note-taking or retaining information, trouble meeting deadlines due to executive functioning difficulties, or needing extra time for exams. Mobility limitations might make it difficult to navigate campus or attend in-person classes, while sensory sensitivities could impact focus in noisy or bright environments. Chronic pain or fatigue may affect attendance and stamina for academic tasks. Providing specific challenges will help us better understand and support your needs.
Students may submit an academic accommodation request at any time. Please note that requests may take approximately one week to process after submitting a Public Accommodation Request, and accommodations are not retroactive.
If you have documentation, such as an IEP, 504 plan, and/or a letter from a healthcare or educational provider, please attach it to the Documentation section of this form. We understand that some students may not have access to documentation for various reasons. Students may still submit a request and meet with an Accessibility Counselor.
All medical documentation, diagnosis letters, and any IEP/504 plans or related information you wish to share are required to be uploaded to your Accommodate case file. This information will allow us to review your materials in preparation for your interactive registration process meeting. Thank you.
DAISS will review your request and documentation submitted. Your accommodation letter will be emailed to your UAlbany email within 3-5 business days (Monday-Friday). You do not need to schedule an appointment unless you're requesting additional academic or housing accommodations. We recommend all incoming students meet with DAISS.
Please make sure to schedule an appointment with DAISS after submitting this request form. You will receive details on how to book an appointment in a confirmation email. We recommend booking appointments in the Summer to guarantee having accommodations in place for the start of classes in the Fall. If you wait until move in, expect a 2-3 week wait for a meeting with an Accessibility Counselor.
